Register By Mail

You can obtain the form for mail-in voter registration at the following links:

• Utah Voter Registration Form
• Utah Absentee Application Form
• Salt Lake County Voter Registration Form (available in both English and Spanish, as required by Section 203 of the Voting Rights Act).
• National Voter Registration Form (available in both English and Spanish, as required by Section 203 of the Voting rights Act).

There is also a voter registration form in The White Pages (located at the end of the blue government section). This form can be photocopied.

Once completed, mail the form to your county clerk's office.

All documents must be postmarked 30 days before the election to be registered for the upcoming election.
